head office
(scanned copy through email will NOT be accepted).
You can also collect and submit this form at any of our
branches
Your online account will NOT be activated until we have received the signed copy of this form.
This has been done for increased security of your
BO account
as well as
the corresponding online account
If your email address is already included in your
BO account
, you don't need to fill up the 'Email address inclusion or replacement form'. Just submit the request to register online. After verification your online account will be activated and you will be notified through email.
